Job Posting /General Description
Creative Community Solutions is seeking to find an individual to assume the position of Care Coordinator for their newly developing Kinship Department. Applicants should be dynamic and highly motivated to work in a growing expanding agency whose mission is to provide the best possible care for children and families.
Applicants must be able to work directly with children and families in care coordination activities. This position requires an individual to be very “hands on” and able to work both in an office setting, in clients' homes, relatives' homes, schools, work settings and in collaboration with other agencies and governmental entities.
Applicants should professionally believe that children do better with relatives and kin than in other living situations. Applicants must be able to advocate for the needs of their clients and do whatever is necessary to maintain stabilization in kinship placements. Good crisis intervention skills will be necessary to be effective in this position.
Building strong therapeutic relationships with clients and families is essential to this position. Being able to engage with clients and families to build trusting productive working relationships will be a key task within this position.
